Analysis of the Current Status of Long-Term Care Services for Elderly Patients with Hip Fractures

Abstract
Purpose: This study analyzed the status of long-term care (LTC) services initiated in elderly patients with hip fractures.

Materials and Methods: Patients aged 65 years and older diagnosed with hip fractures were included as subjects. The data from the medical records and information on LTC from the 2005 to 2018 claim database of the Korean National Health Insurance were used. The applicants and beneficiaries for LTC services for elderly hip fracture patients were surveyed, and the status of service usage in beneficiaries was also analyzed.

Results: Among the 80,675 fracture patients, the number of applicants was 14,795 patients, or 18.3%; the number of beneficiaries was 11,020 persons, or 13.7%, giving a recognition rate of 74.7%. Recognition occurred more frequently in the following groups: females, patients over 80 years, residents of Seoul and cities, those whose medical insurance premium quintile was higher than medical benefits, pertrochanteric or mixed hip fractures, those who have undergone surgical treatment, those with geriatric diseases and those without rheumatic disease, those without physical disability, those disabled with brain lesions and showed statistical significance. As for the status of service usage, 25.9% of beneficiaries use institutional care, 54.3% do home care and 19.8% did not receive any care. Home care was used the most, and institutional care tended to decrease, while home care and non-user increased in recent years and showed a statistically significant difference.

Conclusion: From 2009 to 2017, 13.7% received LTC recognition after a fracture occurred, and the recognition rate compared to applicants was 74.7%, with home care used the most. There was considerable demand for LTC services after hip fractures, and it is considered necessary to expand services as one out of four applicants applied but were not recognized.
